All other terms, conditions, and requirements of the original notice remain unchanged.

This notice does not constitute a solicitation and is issued solely for market research purposes in accordance with RFO 10.001.

The Government will not accept unsolicited proposals or binding offers in response to this notice.

Interested parties should continue to submit capability statements as outlined in the original notice by the response date indicated above.

Question 1: Could you please confirm whether any additional documents, such as the proposed Oral Surgeon's resume/CV or credentialing documents, are required with the response at this stage?

Answer 1: No.

At this stage, no resume/CV for the proposed Oral Surgeon, nor any credentialing documentation (e.g., state license, DEA registration, board certification, malpractice history, or privileging records), is required to be submitted in response to this Sources Sought Notice.

At the Sources Sought stage, the Government is assessing general firm capability and experience, not evaluating specific personnel.
