For Andersen Air Force Base, Guam: 1)Base Access List Instructions (Attachment JL-11). 2)Complete Base Access List (Attachment JL-11). 3)Complete Base Access Affidavit (Attachment JL-11). 4)It is the responsibility of all interested attendees to follow up and make sure NSAA base passes are processed and ready for pick up. 5)Those that do not confirm their base access requests may not be able to participate.
