75H70726Q00107 is issued as a Request for Quotation (RFQ) for one (1) Nurse Practitioner to provide non-personal outpatient primary care services for the Ute Mountain Ute Health Center (UMUHC) located in Towaoc, Colorado.
This acquisition is set aside for Indian Small Business Economic Enterprise (ISBEE) concerns.
This solicitation incorporates provisions and clauses by reference and in full text.

(iv) Services shall be provided at the following Albuquerque Area Indian Health Service (IHS) facility: Ute Mountain Ute Health Center 232 Rustling Willow Street Towaoc, CO 81334 The period of performance consists of a one-year base period and four (4) one-year option periods.
