THE SERVICES OF A STATE OF FLORIDA REGISTERED CONTRACTOR TO PROVIDE ALL REQUIRED MATERIALS, TOOLS, PERMITS, EQUIPMENT, TRAFFIC CONTROL, LABOR AND SUPERVISION FOR THE CONSTRUCTION OF THE GLENDALE STREET TRAIL FROM NEW JERSEY ROAD TO LAKELAND HIGHLANDS ROAD. THE CONTRACTOR SHALL BE PRE-QUALIFIED BY THE FLORIDA DEPARTMENT OF TRANSPORTATION IN THE FOLLOWING WORKGROUPS: (07) DRAINAGE, (10) FLEXIBLE PAVING, (28) PAVEMENT MARKING, (38) ROADWAY SIGNING, AND (40) SIDEWALK AND OTHER WORK CLASSES. THE IMPROVEMENTS FOR THIS PROJECT ARE AS SHOWN IN THE DRAWINGS PREPARED BY THE CITY OF LAKELAND PUBLIC WORKS ENGINEERING DEPARTMENT (FPID: 453404-1-58-01 AND CITY OF LAKELAND PROJECT NO. 008202) AND GENERALLY CONSISTS OF CONSTRUCTION OF A 10-FOOT WIDE SHARED-USE PATH ALONG GLENDALE STREET FROM NEW JERSEY ROAD TO LAKELAND HIGHLANDS ROAD. THE PROJECT INCLUDES CLEARING AND GRUBBING, EARTHWORK AND GRADING, ROADWAY RECONSTRUCTION, SIDEWALK, DRIVEWAY CONSTRUCTION AND DRAINAGE IMPROVEMENTS. THE SUBMITTALS RECEIVED AND THE CONTRACT, IF AWARDED, SHALL BE IN COMPLETE ACCORDANCE WITH THIS REQUEST FOR PROPOSAL AND THE PROJECT PLANS AND SPECIFICATIONS, AS PREPARED BY THE CITY OF LAKELAND PUBLIC WORKS ENGINEERING DEPARTMENT FOR THE CITY OF LAKELAND AND THE FLORIDA DEPARTMENT OF TRANSPORTATION, WITHOUT LIMITATION, AND ALL CODES, REQUIREMENTS, AND REGULATIONS REFERENCED THEREIN. IN ADDITION, FEDERAL AND STATE FUNDING IS BEING UTILIZED UNDER A LOCAL AGENCY AGREEMENT AND WILL BECOME PART OF THE REQUIREMENTS.
