Dear Prospective Offeror, Attached is a Request for Quotes (RFQ) SF 1449, for Bread Products for the Federal Medical Center, Rochester MN.

The Government reserves the right to make formal single or multiple award contract(s) from this solicitation.

Awards will be made based on price and past performance, with pricing being rated higher than past performance.

All potential quoters are advised that this solicitation includes the clause FAR 52.212-4 for Commercial Items, which at paragraph (t) System for Award Management (SAM), requires all contractors doing business with the Federal Government to be registered in the SAM database.

Quoters should include their Unique Entity Identity (UEI) number in their quotes.

The Contracting Officer will verify registration in the SAM database prior to award by entering the potential awardee’s UEI number into the SAM database.

Failure to complete the registration procedures outlined in this clause may result in elimination from consideration for award.

Ensure package size and ounces per package are included in your quote.

The Award will be acceptance of your offer: therefore, ensure all requirements can be satisfied in terms of quantity required, delivery dates, specifications, packaging, labeling, and invoicing procedures, etc., prior to bidding on the item(s).

Do not change specifications or packaging, unqualified quotes will be considered not-responsive.

Quotes must be received no later than the date stated on the SF 1449 , Block 8 .
