The Defense Health Agency (DHA) intends to negotiate a sole source contract under the authority of FAR § 12.102(a) and 41 U.S.C.

3304 with Philips North America LLC (Philips), 222 Jacobs St.

East Cambridge, MA, 02141-2296, for the provision of patient monitoring system maintenance and original equipment manufacturer (OEM) parts for currently installed Philips equipment for the Naval Medical Center Portsmouth, VA (NMCP).

The period of performance is for a period of 60 months broken out in one12-month base period, with four 12-month option periods.

The Contractor shall provide OEM maintenance support and coverage for all equipment listed in Exhibits 2 and 3.

The Equipment listed in Exhibits 2 and 3 will change based on equipment becoming End of Service Life (EOSL) and equipment coming off warranty.

Support of the devices stated in Exhibit 2 shall include the supply of all parts and over the phone maintenance support.

Support for the devices stated in Exhibit 3 shall include telephone support, on-site field support, and system hardware and software updates, as mandated by the OEM, in accordance with the PWS.

All parts, labor, and travel shall be included.

The Contractor shall maintain all equipment listed in the PWS.
