SOURCES SOUGHT NOTICE Indian Health Service — Shiprock Service Unit Health Promotion & Disease Prevention (HPDP) Wellness Program Coordination Services Northern Navajo Medical Center (NNMC), Shiprock, New Mexico The Indian Health Service (IHS), Shiprock Service Unit is issuing this Sources Sought Notice (SSN) to identify capable Indian-owned economic enterprises — large (IEE) or small (ISBEE) — that can provide professional wellness coordination and health promotion services in support of the Northern Navajo Medical Center (NNMC) Health Promotion and Disease Prevention (HPDP) program.

Services include wellness facility access for referred IHS patients and community beneficiaries, community-based health promotion event support, mental health wellness initiative support, health education training, culturally appropriate nutrition and cooking programs, concussion management equipment training support, and crisis prevention and de-escalation training for school staff within the Shiprock Service Unit.

Other capable firms are also invited to respond to this SSN.

The Contracting Officer will consider responses for Buy Indian Act (25 U.S.C.

47) opportunities first, then for set aside programs under the Small Business Act (41 U.S.C.

3104).

Background and Description of Requirement The Shiprock Service Unit of the Indian Health Service serves a large and geographically dispersed Navajo population across 20 outlying chapters.
