This is a Sources Sought Notice only This is not a solicitation, request for proposal (RFP), or request for quotation (RFQ). This notice is issued solely for market research purposes in accordance with FAR Part 10 to determine the availability of qualified Indian Economic Enterprises (IEEs) and Indian Small Business Economic Enterprises (ISBEEs) capable of meeting the Government's requirements. The Indian Health Service (IHS), Navajo Area Office, is seeking information from firms interested in providing National Diabetes Prevention Program (National DPP) Lifestyle Coach Training using the CDC-recognized PreventT2 curriculum. Background The Navajo Area Indian Health Service provides healthcare services to more than 200,000 members of the Navajo Nation throughout Arizona, New Mexico, and Utah. Through the Special Diabetes Program for Indians (SDPI), the Navajo Area seeks to increase local capacity for diabetes prevention by training healthcare professionals to deliver evidence-based lifestyle intervention programs. Requirement The Contractor shall provide one (1) two-day, in-person National DPP Lifestyle Coach Training for approximately 18 participants utilizing the CDC-recognized PreventT2 curriculum. The Contractor shall: Provide qualified instructors experienced in the CDC National DPP Lifestyle Change Program. Provide all instructor and participant training materials. Conduct the training at a location designated by the Navajo Area IHS. Utilize current CDC-approved PreventT2 curriculum. Provide interactive instruction including demonstrations, group discussions, multimedia, and technical assistance. Provide participant resources and toolkits.
