See attached 36C25626Q01072 and draft Scope of Work Description: The Department of Veterans Affairs (VA), Alexandria Veterans Healthcare System (VHCS) in Pineville, LA is seeking information from potential vendors on their ability for Contractor to furnish all labor, equipment/supplies, management, qualified personnel, travel/transportation and documentation for providing Alexandria VHCS Physical Medicine and Rehab Durable Medical Equipment (DME) service as listed in the attachment. This is for a base of 12 months, with four (4) additional, 12- month options to be exercised at the Government s discretion. Information Requested from Industry: In response to the Sources Sought, interested Contractors shall submit the following information and any Capabilities/Qualifications Statement to include but not limited to an overview of proposed solution(s) and a description of the product your company possesses in accordance with the attached Statement of Work (draft). Responses to attached market research table questionnaire shall be submitted. If this information is not provided, then it will be assumed the entire requirement cannot be met.
